Consent for Storage and Use of Personal Information

The BC Freedom of Information and Protection of Privacy Act ("FIPPA") prohibits the disclosure of personal information outside of Canada without your consent. This consent must be informed and documented in a record. Personal information held by US companies is potentially subject to disclosure demands from US government authorities and that under US law such demands may occur in secret. Please review the following consent provisions below prior to registering for the challenge and entering your personal information.

1. I understand that the PHSA Employee Wellness & Safety is sponsoring an employee health & wellness program to provide staff with the opportunity and tools to change fitness and dietary habits.

2. I understand that the PHSA has partnered with Envision True Health Corporate Services to conduct this program and the information collected on the online tool will be processed and stored in the United States of America.

3. I understand that in order to participate in the program, I will be asked to provide my name, agency, date of birth, email address, height, current and goal weights to Envision True Health Corporate Wellness. This information will not be shared with PHSA; however information on utilization will be sent in aggregate form to PHSA Employee Wellness & Safety.

4. If I accumulate 100 points, and I am selected as a winner of a prize, my name and email address will be provided to Employee Wellness & Safety by Envision True Health Corporate Services.
